The practice area is Alrewas, Fradley, Bromley Hayes, Kings Bromley, Orgreave, Croxall, Wychnor, Catton, Edingale, Harlaston, Lullington, Elford.
Please visit the surgery and our receptionist will give you a registration pack with the relevant forms to complete.

Non-urgent advice: Notice
Your registration pack will include a detailed New Patient Questionnaire.
This enables us to gather information about your current and past health, which is very useful to us as we have to wait for your medical records to come from your previous doctor.
The New Patient Questionnaire will be reviewed and a suitable appointment will be made for you if necessary.
When this has been done and the forms have been returned to us, you will be registered with a GP partner. However, as this is a group practice you will be able to see any of the doctors.
If you have a child under 5, you will be given another form to complete when you come into the surgery
