Summary Care Record
The Summary Care Record (SCR) is used in emergency care. The record contains information about any medicines you are taking, allergies you suffer from and any bad reactions to medicines you have had.
Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.
When you join the practice, we upload this basic information and a Summary Care Record is created for each patient registered with us. You also have the option of allowing other professionals to see an enriched Summary Care record if you wish. This includes additional information from your patient record and includes significant medical history, information on any long term conditions you may have, your immunisation record, your communication preferences and any end of life care information.
For further information you can:
- download and read the following documents SCR Additional Information Leaflet (click here) or SCR Easy Read Patient Leaflet (click here)
- access the Summary Care Record website
- call the Summary Care record Information line on 0300 303 5678
You have the choice to have a summary care record, an enriched summary care record, or to opt out.
Whether you choose to have a Summary Care Record now or not, you can change your mind at any time by informing us at the surgery.

When a patient/client visits any of the organisations listed below, or use services provided by them, staff directly involved in their care will have access to the important information from health and social care records.
- Staffordshire and Stoke-on-Trent GP practices
- University Hospitals of North Midlands NHS Trust (Royal Stoke)
- University Hospitals of Derby and Burton NHS Foundation Trust
- Midlands Partnership NHS Foundation Trust
- North Staffordshire Combined Healthcare NHS Trust
- Staffordshire County Council (Social Care)
- Stoke-on-Trent City Council (Social Care)
- Continuing Healthcare services

You have the choice to opt out of this service if you wish.
If you wish to have this record, you do not have to do anything. One will be created for organisations listed above to see.
