We have introduced an on-line appointment booking facility via Patient Access.
You will be able to book appointments for all the Doctors on-line. Appointments for blood tests, appointments with a nurse and extended access appointments must still be booked through reception.
In order to access this system, patients will need to request individual login details from the surgery. This should be done in person as we will require all patients applying to complete a form and to provide identification and proof of current address.
For security reasons, the information required for you to register for the on-line booking system will always be posted to you at the address where you are registered with the surgery. Wait for this to arrive before going to Step 3.
Once you receive your information form in the post, you can register on the Patient Access system by following the instructions on the sheet. You will need to use this information the first time you access the system – subsequently, you will only need to sign in with the details you have created.
Before you can use the online booking system, Patient Access, you need to register.
Please print out and complete the following form and return it to the surgery with appropriate ID to either firstname.lastname@example.org or drop into our letterbox at the surgery.
When booking an appointment on-line, please ensure that it is essential for you to see a Doctor. Many appointments can be dealt with by our Practice Nurses or Health Care Assistants or in a Telephone Consultation.
A standard appointment time with a Doctor is 10 minutes – if you feel you need longer than this, please book your appointment through reception.
Appointments with the Practice Nurses and Health Care Assistants
Due to the variety of treatments offered by the nurse team, appointments need to be booked through reception to ensure you have the correct length of appointment you need. Therefore, these will not be available to book on-line.
Our Practice Nurses provide a wide range of services including: cervical cytology, contraception, advice on family planning, minor injuries, dressings, immunisations, travel advice, suture removals and long term conditions clinics (such as Diabetes, Asthma, Heart problems). You can also see our Nurse Practitioners for minor ailments such as: urine infections, coughs, colds, rashes and backache.
Our Health Care Assistants provide a supporting role to the clinical staff, including: taking blood tests, INR testing, NHS Healthchecks, smoking cessation and 24hr BP monitoring.
If it is more convenient for you to speak to a doctor or nurse by telephone rather than visiting the surgery, please contact reception to make a telephone appointment.
